Cor Medical - On-Site Health Clinic

Lumen is pleased to offer an on-site health clinic at its company headquarters location in Monroe, LA, Littleton, CO and Broomfield, CO.

Lumen partnered with Cor Medical to open a work-site employee medical clinic at the company’s headquarters in 2016. Since that time, employees have benefited from the convenience, expertise, and a variety of health services in Monroe, Littleton, and Broomfield. Cor Medical serves employees, spouses, domestic partners, and dependents (ages 18-26) enrolled in a Lumen medical plan. Cor Medical offers immediate primary care (for those without respiratory symptoms) in the clinics or through virtual visits. Cor Medical provides mental health services in the clinic or through virtual visits. To book your appointment, visit portal.cormedical.net.

The clinic is managed by Cor Medical, a privately-owned independent company whose mission is "to provide easier access to effective and high-quality primary care for employees while minimally impacting their productivity, time off work, or time away from family by locating the clinic at or near your workplace."

Who is eligible?

ALL U.S. Lumen employees, Spouses and Dependent Children (ages 18-26) enrolled in one of our medical plans are eligible to use the clinic (Please note: Employees do not have to "work" at the Monroe, LA, Littleton, CO or Broomfield, CO locations to use the clinics. Employees travelling to these offices covered under one of our medical plans are eligible to use the clinics as well).

Cor Medical Services

Lumen's Monroe, Littleton and Broomfield employees, enrolled in a Lumen medical plan may now schedule virtual appointments with Cor Medical. Simply log into (or create an account) with Cor Medical, and select the Patient Resources link on the right sidebar for instructions on how to set up a virtual visit by mobile device, laptop or desktop.

Same day or next morning pharmacy delivery (Through a local pharmacy partnership, your prescriptions can be delivered to Lumen's on-site health clinic).
Prescriptions can also be sent electronically to any pharmacy of your choosing

Cost and claims information

Your share in the cost of the clinic is as follows:

If enrolled in a CDHP (Option 1 or Option 2): Zero dollar copay
If enrolled in the HDHP: Full cost of visit/services until deductible is met
If enrolled in the Bind Health Plan: Zero dollar copay

Payment options include: Cash, personal credit card, personal debit card, or HSA debit card.

To obtain treatment please present your Lumen badge and United Healthcare medical ID card at the clinic and pay your copay, if applicable..

Family Nurse Practitioner, Cyndi Peschong, is virtually available for all of your health and wellness needs Mon-Fri, 8a.m. to 5p.m. (MST). Onsite Nurse Practitioner days are to be determined at this time.
