Important: Lumen’s plan provisions define "Medicare eligible" as individuals age 65 or older, and/or Medicare eligible, including individuals under age 65 who qualify due to a disability or ESRD.

Pension Plan Summary Plan Descriptions: You may read and print the applicable SPD for your employment history online. You also have the right to request a paper copy by contacting the Lumen Pension Service Center at 888-324-0689, Monday to Friday, 8 a.m. to 7 p.m. CST. Press option 2, then option 3 when prompted to be connected to a Service Representative.
