• Are a CenturyLink Union Represented Employee, as defined in the glossary of a Participating Company (please refer to the SPD below)
• Are a CenturyLink Union Represented Employee, who is full-time and whose standard hours are 30 or more hours per week
• Are a CenturyLink Union Represented Employee, who is part-time and whose standard hours are at least 20 per week and whose collective Bargaining Agreement provides eligibility for part-time employees (please refer to Appendix I of the SPD below)
• Have completed the service requirement, which is: One (1) year unless your respective governing Collective Bargaining Agreement indicates otherwise (please refer to Appendix I of the SPD below) and satisfy all other Plan eligibility requirements.
