Overview

Improving or maintaining your health is often easier when you have support from a spouse or domestic partner. Spouses/DP covered on a Lumen medical plan are eligible to participate in several wellness programs and can even earn Rally rewards and receive fitness reimbursement. Support each other and get healthy together!

Getting started

Spouses enrolled in a Lumen medial plan can begin on Rally by registering on the Rally health app or at lumen.com/wellconnected. To register, the spouse will register with their personal email address and the employee's SSN. Spouses can then begin completing activities on Rally just like employees to begin earning the annual $600 reward.

Reminder: the Rally health survey and biometric screening are both required to earn all other rewards.

Well Connected Rewards

Rewards for spouses will be available based on the medical plan that the employee enrolled in. Spouses / domestic partners can now participate in Choice Rewards allowing you to change and choose how you redeem rewards as they are earned.

Please note: rewards redeemed as contribution to a HRA or HSA will be distributed to the employee's account.

Taxes on Rally Gift Cards:

If the employee and spouse elect to receive Rally rewards in the form of gift cards, those rewards are considered taxable income and therefore will be taxed. Social Security and Medicare taxes will be withheld from the employee's paycheck when the rewards are redeemed. Social Security taxes (6.2%) and Medicare taxes (1.45%) for anyone with taxable Medicare earnings in excess of $200,000, an additional 0.9% will be withheld from your paycheck.

At the end of the year, Wellness Rewards income as of November 30th, will be included on the employee's W-2 statement. Employees will be responsible for paying federal, state and local taxes related to earned rewards on their tax return.

Any Wellness Rewards earned during December will carry forward to be applied to the employee's next year's taxable income.

Fitness Reimbursement

Spouses / domestic partners enrolled in a Lumen medical plan - can receive fitness reimbursement of 50% monthly membership cost, up to $25 per month. The employee is required to enter the spouse's fitness reimbursement through the fitness reimbursement intranet page located on the wellness page. The reimbursement will be added to the employee's paycheck.

Taxes on Fitness Reimbursement:

Reimbursement received for spouse's fitness reimbursement is considered taxable income. Social Security and Medicare taxes will be withheld from the employee's paycheck when the fitness reimbursement is deposited to the employee's paycheck.
