Flu Shot Voucher Program

To help you stay healthy, Lumen offers five different options for receiving the flu shot. This is offered in the Fall of each year. All of these options are available at no cost to you.

The three options to receive a free flu shot are:

  • Cor Medical onsite clinics.
  • Cor Medical in Broomfield, CO and Monroe, LA provides free flu shots to all Lumen employees, their spouses/domestic partners and dependents (age 18-26) enrolled in a Lumen medical plan. Schedule an appointment or visit one of their drive-thru flu shot events each fall.
  • Voucher Program
  • Employees who do not have access to or are unable to attend an onsite flu shot clinic can print a Walgreens flu shot voucher and take it to a nearby Walgreens pharmacy for a free flu shot.
  • Walgreens flu shot vouchers are preferred for Lumen employees. Spouses/dependents on a Lumen medical plan may receive a free flu shot through retail pharmacy participation and through primary care.
  • Retirees or spouses of retirees who are on a Lumen medical plan can print a Walgreens Retiree voucher and take it to a nearby Walgreens pharmacy for a free flu shot.
  • Physician's office or pharmacy
  • Employees, spouses/partners, and dependents enrolled in a Lumen medical plan.
  • Schedule a visit with your physician to receive a flu shot.
  • For UnitedHealthcare members, employees and their benefit-eligible dependents can receive a flu shot at several Participating Pharmacies or Convenience Care Clinics (including Walgreen's). The location will process the claim through your medical benefits.
  • For Surest (formerly Bind) members: Show your OptumRx pharmacy ID card at any participating network pharmacy that provides flu shots, and the pharmacy will process the claim through your pharmacy benefits.

The vaccine begins to protect you after one to two weeks, and protection may last up to one year. If you have specific medical questions, contact your physician.

Vaccination is recommended for everyone six months and older, according to the Centers for Disease Control and Prevention (CDC).

Important note: Please contact your physician to discuss whether a flu shot is recommended for you. If you choose to get your vaccine at your physician's office, you may have to pay for the office visit. Certain states do not allow minors to receive vaccines at the pharmacy. Please check with your local pharmacy to determine any age restrictions in your state.
