If you have already been diagnosed with one of these chronic health conditions you can utilize the resources below to help you better manage your condition.

United Healthcare Health Advocate

The United Healthcare Health Advocate team is made up of registered nurses and lifestyle coaches to help Lumen employees and their families navigate the health care system and provide education and support on almost any health care need.

If you have been newly diagnosed, or already managing a chronic health condition, the United Healthcare Health Advocate team can connect you with a condition management program for any of the following: asthma, diabetes, coronary artery disease, heart failure or Chronic obstructive pulmonary disease. Access to this team in available at no cost to employees, spouses, and dependents enrolled in a United Healthcare medical plan through Lumen.

Better Choices, Better Health Condition Management Program (Bind members only)

If you are enrolled in the Bind Plan, you may also receive outreach from a Bind case manager. Bind Case Management helps members that are living with a health condition optimize their health and health care benefits by helping to coordinate care through the development of individualized care plans to meet your needs. Bind's RN case managers ensure that you have the necessary tools and support to achieve optimum health and improved self-management in the most appropriate setting.

2nd.MD

2nd.MD is a virtual platform that allows members to connect with leading medical experts for expert medical opinions related to ALL CONDITIONS such as: cancer, heart disease, digestive issues, and mental health problems. 2nd.MD's care team can help find the right doctor for you. All of this is done from the comfort of your home - no travel required!

Diabetes Programs

  • DayTwo: Employees enrolled in a UHC plan; a science backed Diabetes health program that empowers people by providing food as medicine as an approach to manage glucose levels and improve overall health.
  • Virta: Employees enrolled in the Bind Health Plan can access Virta, an interactive Diabetes coaching program focused on helping people reverse type 2 Diabetes.

Cancer Resources

  • Cancer Resource Services: If enrolled in a UHC plan, and preparing for cancer treatment or have already started, an oncology nurse can help you navigate treatment options and help you find a network provider from a high-quality Centers of Excellence (COE) facility.
  • MyCancer Journey: If enrolled in the Bind Health Plan MyCancer Journey offers support to assist you in choosing the right treatment path.

Virtual Physical Therapy

  • Hinge Health: Employees enrolled in a UHC or Bind Plan have access to this free, fully remote program designed to help you with back, muscle, and joint care. Employees and dependents ages 18+ enrolled in a Lumen medical plan are eligible to participate.
  • Kaia: If enrolled in a UHC plan, Kaia offers a next generation care solution for back and joint pain, delivered on-demand and available 24/7 through a mobile app on your smartphone or tablet.

Lumen offers weight loss program and resources for those looking to make healthier choices and lose weight.
