- Employee Assistance Program: 100% coverage for up to 8 visits. - Preventive Medical Care: 100 percent coverage (in-network) for annual screening tests for cancer, heart disease, osteoporosis, and other diagnostic tests based on medical guidelines. Refer to the list of preventive screenings tests covered by UnitedHealthcare and Bind medical plans.
- Preventive Dental Care: 100% coverage for exams, cleanings and x-rays twice per year. Application of fluoride and sealants for children are also covered at 100%. Available in both MetLife plans.
- Preventive Vision Care: 100% coverage after a $10 copay for an annual eye exam. Available in the EyeMed vision plan.
- Tobacco-Free Household Discount: You may be eligible for a discount on the total cost of medical coverage if you and your eligible dependents enrolled in a Lumen medical plan are non-tobacco users or are enrolled in a company-recognized tobacco cessation program.
- Healthy Pregnancy Program: is an education program to ensure a healthy delivery. You can get personalized help through pregnancy and delivery with healthy pregnancy programs available through UnitedHealthcare and Bind medical plans.
Other onsite programs available in the Lumen Wellness Program:
- Fitness Reimbursement: Reimbursement of 50 percent, up to a maximum of $25, per month for eligible fitness expenses like memberships, class fees, subscriptions or events and activities.
- Fitness Centers: In some locations, on-site fitness centers and/or equipment are available.
