Amazing Benefits at Lumen

Lumen offers a broad range of Health, Welfare and Voluntary Lifestyle benefits that enhance your physical, mental, emotional and financial wellbeing. Beyond the comprehensive benefits package, employees are eligible for additional benefits and perks (such as Employee Concessions, Advocacy Services and more).

Programs listed below are offered to U.S. based employees only.

Important Note: Union Represented employees may not be eligible for all of the benefits listed below. Please refer to your Collective Bargaining Agreement (CBA) for more information.

  • Core benefits
    Medical

    Lumen offers consumer driven health plans (CDHPs) with a company-funded Health Reimbursement Account (HRA); High Deductible Plan (HDHP) with an optional employee funded Health Saving Account (HSA) administered by UnitedHealthcare and the Bind Health Plan.

    Prescription Drug Program

    Lumen offers you and your eligible dependents a comprehensive prescription drug plan through UnitedHealthcare and Bind Health Plan with OptumRx.

    Dental

    Lumen employees have a choice between two dental plan options: Dental Plan Option 1 and Dental Plan Option 2 with orthodontia. Both are administered by MetLife.

    Vision

    The Lumen vision care benefit is one option and is administered by EyeMed.

    Health Savings Account

    An HSA can only be elected if you enroll in the High Deductible Health Plan (HDHP) with optional Health Savings Account (HSA). Annual contribution limits are determined by the IRS.

    Flexible Spending Accounts

    The Lumen benefits program offers two types of flexible spending accounts -- a Health Care FSA and a Dependent Day Care FSA. These optional pre-tax accounts are funded by you and can help you pay for certain expenses not covered by other benefit plans.

    Telemedicine

    United Healthcare participants

    • MDLIVE - MDLIVE provides anytime, anywhere access to quality healthcare from local physicians who can diagnose, treat, and write prescriptions for routine medical conditions. With MDLIVE, you can access a board-certified doctor via secure online video, phone or the MDLIVE App - 24/7/365.
    • UHC Virtual Visits - If you are seeking treatment for illnesses like the flu, allergies, pinkeye and more, a UnitedHealthcare Virtual Visit can provide you with access to care from your home. Doctors are available 24/7 by phone or video. Employees and dependents enrolled in a United Healthcare plan are eligible for this telehealth benefit.

    Bind Health Plan participants:

    • KHealth - If enrolled in the Bind Health Plan, K Health gives 24/7 access to doctors for your routine primary care, acute care, and chronic disease management needs.
    • Doctor on Demand - Bind offers Telemedicine services for employees and their eligible dependent(s) enrolled in the Bind Health Plan for a wide range of common, non-emergent health issues. Virtual Visits through Doctor on Demand let you skip the waiting room for a cheaper, faster option. Talk anytime from anywhere with real, board-certified doctors via phone, chat or video conference and obtain a diagnosis and treatment.
    Commuter Spending Account

    Lumen offers a pre-tax benefit account that can be used to pay for public transit or parking expenses. You can contribute up to $280 monthly (on a before-tax basis).

    Short Term Disability

    When you have medical circumstances that require time off work, Lumen provides Short Term Disability benefits to continue all or a portion of pay to eligible employees when they are disabled.

    Long Term Disability

    Long Term Disability is designed to help protect your income in the event you are unable to work due to a covered disability.

    Life Insurance

    Lumen Life and AD&D Insurance Plans provide a wide range of coverage in the event of death or certain other serious physical losses, such as Basic Life, Supplemental, Basic AD&D, Supplemental AD&D and Travel Insurance coverage for the Employee, Spouse, Domestic Partner or Dependent/Child.

    Business Travel Accident (BTA)

    Lumen’s Business Travel Accident (BTA) Plan provides travel assistance services to help employees with travel mishaps or emergencies during your business trip and additional financial protection in case of accidental death or injury while you are traveling on company business.

    Survivor Benefit

    Survivor will receive six months of your continued pay (as a lump sum) and six months continued health coverage.

    401(k) Savings Plan

    Save up to 80% of your eligible pay and receive a company match up to 4% on the first 6% you contribute. If eligible, an annual “true-up" match is also given. If you are a union employee, please refer to your Collective Bargaining Agreement (CBA) for contributions and company matching.

    Pension

    Company-funded pension benefits for employees who meet certain vesting eligibility requirements.

  • Wellness rewards with Rally
    Rally Rewards

    Complete healthy actions with the Well Connected program and earn rewards:

    Through Rally, all U.S. Lumen employees and their covered spouses/domestic partners can earn up to $600 each year in wellness rewards.

    Rewards can be redeemed for gift cards or contributed to your HRA/HSA depending on your medical plan.

    Reward Type

    The reward you receive is determined by the reward option and medical plan you selected:

    • CDHP: Gift cards or Health Reimbursement Account deposit
    • HDHP: Gift Cards or Health Savings Account deposit
    • BIND Health Plan: Gift Cards
    • Waived Coverage: Gift Cards

    *New in 2022, you are now able to choose and change your reward option throughout the year under specific plans. For example, you can choose to redeem some of your Well Connected rewards as a gift card, then choose to redeem the rest as a deposit into your health account.

    Talkspace is an online therapy platform that offers access to licensed therapists who cover a broad range of mental health services.     Rethink

    Rethink provides family support when you need it. Their research-based program helps families raising children with learning, social or behavioral challenges, or developmental disabilities. This program is provided by Lumen at no cost to you.

    To promote employee health and wellness, Lumen will reimburse 50% up to a maximum of $25 each month (maximum $300/ per person per year) for eligible fitness expenses. All full-time employees, as well as spouses/domestic partners enrolled in a Lumen medical plan are eligible.

    Weight Watchers

    Reimbursement: Employees and spouse/domestic partners who are enrolled in a UHC or Bind medical plan option will be eligible to receive up to $55/month for participating in a Weight Watchers Program. A prescription from your health care provider is required advising of a weight management related medical condition or illness (e.g., heart disease, obesity, hypertension), along with a receipt and a Weight Watchers Reimbursement Form.

    Rally Rewards: Employees and eligible spouse/domestic partners will be able to earn a $50 Well Connected reward if you complete 12 weeks of online or in person Weight Watchers coaching; or lifetime members can receive the $50 reward after they complete 3 monthly weigh-ins.

    Transplant Resource Services

    Provide high-quality transplant care at an affordable cost. The Centers of Excellence network offers unmatched economic performance at the nation’s leading clinical institutions. High-Quality, Specialized, Complex Transplant Care.

    Neonatal Resource Services (NRS)

    NRS works to identify pregnancies that may be high risk, and to encourage delivery at a Neonatal Centers of Excellence Network facility.

    Cancer Resources

    Cancer Resource Services: If enrolled in a UHC plan, and preparing for cancer treatment or have already started, an oncology nurse can help you navigate treatment options and help you find a network provider from a high-quality Centers of Excellence (COE) facility.

    MyCancer Journey: If enrolled in the Bind Health Plan MyCancer Journey offers support to assist you in choosing the right treatment path.

    ABA Therapy

    ABA Therapy is a type of intensive behavioral treatment for Autism Spectrum Disorder. ABA treatment is generally focused on the treatment of core deficits of Autism Spectrum Disorder, such as maladaptive and stereotypic behaviors that are posing danger to self, others or property, and impairment in daily functioning.

    Advocacy Services

    2nd.MD

    A Second Opinion service that allows members to connect with leading medical experts for expert medical opinions for all healthcare conditions including spine and joint, cancer, heart disease, digestive issues, mental health problems and more.

    Lumen requires that you consult with 2nd.MD prior to a hip, knee, shoulder or spine surgery (on a non-emergency basis).

    Healthy Pregnancy Program

    UHC Maternity support: Employees enrolled in a United Healthcare plan have access to support (by phone) from a nurse with experience in obstetrics when you enroll in the Healthy Pregnancy Program. They will develop an exercise and nutrition plan, provide hospital and provider information, develop a birth plan, and plan for upcoming appointments.

    Pacify Plus: Employees enrolled in the Bind Health Plan have access to Pacify Plus to support you and your baby for a healthy delivery Work with a dedicated nurse to prepare for your appointments, delivery, and post-partum.

    Fertility Programs

    Fertility Solutions: Employees enrolled in a UHC plan will have access to Fertility Solutions. This program will connect you to an experienced fertility nurse who can provide information about treatment options and lend support.

    Progyny: Employees enrolled in the Bind Health Plan have access to Progyny a comprehensive fertility and family building Benefit, leveraging the latest technologies and treatments.

    Preventive Screenings

    100% coverage for in-network services as allowed under the current recommendations of the United States Preventive Services Task Force.

    Diabetes Programs

    Bridging of Service

    The Bridging of Service Policy grants service credit in certain situations to rehired employees who were either previously employed by Lumen or its predecessor company, CenturyTel, Inc. or whose service was recognized by an acquired company or via an asset purchase agreement. Service is recognized so that equitable action can be taken regarding paid-time off or vacation (for employees in positions graded P3 or below), layoffs, severance allowances, approved absences, short- term disability and service awards.

    Lumen offers Tuition Reimbursement Programs in support of U.S. employees' efforts to increase their effectiveness in their current positions. Up to $5,250 reimbursed to regular full time employees.

    College Coach (Non-Union only)

    Take the worry out of your child’s educational future with Bright Horizons College Coach. This free benefit gives you access to a team of college admissions and finance experts who can help you maximize your child’s chances of admissions success and plan for college costs.

    Through College Coach, you have access to Rev Prep, which offers six free months of Homework Help. This benefit can be used alone or as a companion to their regular tutoring package, which Rev Prep offers at a 10% discount.

    Adoption Assistance

    Lumen offers Adoption Assistances for any legally adopted child under the age of 18. This includes stepchildren who are adopted by an employee or the employee’s spouse. Benefit amount - 100% up to $5,000 per child or $6,000 for a child with special needs.

    Employee Concessions

    Lumen employees (Active and/or Retiree) who live in a Lumen consumer service area and subscribe to a Lumen Service may be eligible to receive a concession (discount) on qualifying Lumen services.

  • Time away from work
    Paid Time Off (PTO)

    The Paid Time Off (PTO) program provides non-represented, regular full-time and regular part-time employees in positions assigned to grades P3, M2, and below paid time when they are away from work for rest and relaxation, short-term illness or injury, to care for family members or for other personal or family matters.

    Flexible Time Off (FTO)

    The Flexible Time Off (FTO) program provides non-represented, regular full-time exempt (salaried) employees in eligible positions, assigned to grades P4/S3/SS3/SE3/I16 or above time away from work for rest and relaxation, occasional sick days for themselves or to care for family members, or other personal business. The Flexible Time Off (FTO) plan does not include a set amount of time that can be taken off. Instead, you and your manager will mutually agree when time can be taken off, based on both your needs and the needs of the business. There's no time to track or balances to manage or carry over.

    The policy is intended to empower employees to balance personal lives with demands of the business and to create a reciprocal relationship between the Company and employees.

    Holidays

    Lumen provides eligible employees paid time off from work to observe a number of standard US holidays.

    Religious Observances

    Lumen allowed reasonable time off if required for a religious observance or practice if it does not place an undue burden on the business. Approved absences for a religious observance or practice are excused for the purposes of the attendance policy and coded to PTO/FTO.

    Bereavement

    Lumen is committed to providing a compassionate work environment. To that end, for non-represented, US based employees the Company will provide paid time off for bereavement for certain regular full-time and part-time (pro-rated based on hours normally worked).

    Your Own Disability

    Lumen provides disability and leave benefits to eligible employees who have special circumstances that require time off from work. These benefits may include continued pay, medical and other benefits for those who meet eligibility requirements.     Maternity Leave

    Provides a paid, approved absence for an eligible expectant mother through our STD benefit program.

    Parental Leave

    Provides a paid, approved absence for eligible a new birthmother, non-birth mother, and father for the birth & care of a newborn child.

    Family and Medical Leave

    Family and Medical Leave Act (FMLA) is federal legislation designed to protect employees in the event there is a serious health condition; an employee may need to care for an eligible family member with a serious health condition; or they need bonding time for the birth, adoption, or foster placement of a child. Sedgwick administers FMLA for all employees in all states.

    Military Leave

    Provides excused, sometimes paid, leave for our uniformed service employees for Drill, Annual Training, Active Duty for Training or Active Duty.

To help fulfill this commitment, we have partnered with our disability insurer - MetLife - as part of our Disability plan, to provide assistance to employees who have a medical condition that could lead to a disability and to those who have already filed a disability claim. The Consultants assist our employee with staying at work by arranging ergonomic reviews and providing adaptive technologies, as well as, returning to work from STD. This is done by removing barriers, assessing work stations, helping employees resume job duties and promoting communication among all parties involved. This service is called the Early Intervention Program.

    Nursing Mother's Rooms

    Nursing mothers will be provided reasonable break time during the work day to express breast milk. Nursing Mother's Rooms are provided in various locations.
